ELM provides services in your own home and in your community — not in group homes or congregate-care facilities. Every service is delivered in alignment with your individualized support plan.
Direct support services
ELM staff support you in your own home and in your community, in alignment with your individualized support plan. Our direct support offering includes:
ELM operates a structured on-call system. A designated on-call staff member is available 24/7. You will receive, in writing, the after-hours contact number, a description of what constitutes an emergency, the list of external emergency resources (911, mobile crisis, hospital), and the expected response timelines.
Individualized Funding management
ELM can receive, hold in trust, administer, and report on your Individualized Funding allocation in accordance with your funding agreement with the Province. We support self-management, ELM-managed, or blended IF arrangements — whichever you prefer.

- A written IF Management Plan.
- Monthly financial statements detailing expenditures against approved funding categories.
- Quarterly budget reviews to assess variance and reallocation needs.
- Support for self-management, ELM-managed, or blended IF arrangements — whichever you prefer.
What we don't provide
ELM is clear about its scope of practice. We do not replace clinical, medical, or specialized professional services. If you need those services, ELM staff support warm referrals, help with applications and navigation, and document follow-up — but ELM staff do not deliver those services directly.
ELM is not specialized in supporting participants whose primary barriers are physical or mobility impairments. If that’s true for you, we will be honest about our scope and will work with the Department to identify a better-fitting provider.
